デルモリポーマ

概要

• 分離腫に分類される先天良性腫瘍
• デルモリポーマは上耳側の結膜円蓋部に発生することが多く、結膜デルモイドと呼ばれることもある
• 病理組織学：密な結合組織に囲まれた成熟脂肪組織からなり、毛、皮脂腺、汗腺などを含んだ重層扁平上皮でおおわれている
• oculo-auriculo-vertebral spectrumに合併することがある

治療

• 治療は経過観察されることもあるが、腫瘍が瞼裂より露出したり、異物感などの自覚症状がある場合は手術適応となることがある
• 過剰な切除やそれに伴う瞼球癒着によって、術後に眼瞼下垂、複視を伴う眼球運動障害、乾性角結膜炎などの合併症を生じることがある

• 結膜を温存した腫瘍部分切除による合併症の回避や、結膜移植などによる再建の有用性が報告されている
